After the Storm Comes New Life Part 1

I hereby grant the following rights to JOY MINISTRIES AND DANETTE CRAWFORD MINISTRIES (DCM). and any of its affiliates, licensees, subsidiaries, or assigns in consideration of their possible use of my (and my childs) visual depiction by photographs, video, digital images or transparencies, any oral statements, and any other information supplied by me (collectively, the “material”). I acknowledge that no promise or representation has been made to me that Joy Ministries/DCM shall be obligated to use the material in any way, and I acknowledge that Joy Ministries/DCM shall have sole and absolute discretion and creative control in determining when or whether the material should be used in any manner.

Joy Ministries/DCM shall have total ownership of the material in perpetuity and throughout the world; the right to edit the material; the right to print the material; the right to broadcast, exhibit, distribute or display the material on broadcast or cable television, satellite transmission, films, photographs, videotapes, videocassettes, videodiscs, print and digital media, the Internet, radio format, record album, audiocassette format, or by any other method or device now known or hereafter devised; the right to copyright the material; and the right to transfer and assign any or all of the rights hereunder to others, or to license others to use these rights. Joy Ministries/DCM may use my name, likeness, biographical information and/or other materials supplied by me for purposes of advertising, publicity and promotion, but not as a direct endorsement for any product or service. I affirm that the use of my likeness and/or materials supplied by me as described above will not violate the rights of any person or organization and will not incur any liability for payment to any person. I acknowledge that

I will not receive any compensation for the use of such material and I further agree to hold Joy Ministries/DCM harmless from any and all liability that Joy Ministries/DCM may incur as a result of its use of the material as stated herein. Joy Ministries/DCM has right to use the material for the next 25 years and indefinitely. I consent for my child(ren) to participate in the Program and the above-described media and agree to be fully bound by the terms and conditions of this Agreement, as stated above, on behalf of myself/ourselves as his/her parent(s)/guardian(s). I/we agree that the provisions of this Agreement, including, but not limited to, the provisions relating to the release to DCM/Joy Ministries is applicable to me/us and that I/we are bound thereby. I/we further agree to indemnify and hold DCM/Joy Ministries harmless from and against any claims asserted by my/our child. No oral representations, statements or inducements have been made to me/us with regard to this Agreement or the Program.
